Coulby Lodge is a Residential Care Home with Nursing for adults living with a learning disability
Situated in an ideal location on the edge of Hemlington, Middlesbrough, Coulby Lodge supports up to eight adults who are living with a learning disability and complex needs, and who may have an autistic spectrum condition. The team also supports those who have complex behaviours and associated mental or physical health needs.
Coulby Lodge consists of two four bedroom detached buildings, each set in their own private garden. Both buildings have their own living space and kitchen diners, to help promote independent living. All eight bedrooms are ensuite, and the people who use the service are encouraged to personalise and decorate their rooms how they want them to be.
Overlooking the North Yorkshire countryside, the gardens have their own space for relaxing either alone or enjoying other people’s company.
The service is situated in a great location with plenty of community facilities within walking distance, including a shopping centre, large supermarket, community farms, parks, a leisure centre with a large pool and other community activities.
Just a short drive or bus ride away is Teesside Retail and Shopping Park and Middlesbrough Town Centre, Coulby Lodge is on the outskirts of some stunning countryside with Great Ayton and Stokesley just a short drive away. To see a map of the surrounding community facilities, please click here
Coulby Lodge has Support Workers who support service users to have individual activity plans, which are created based on each person’s likes and hobbies. Their goal is to enable people to structure their time, build relationships and establish a balance between therapy, self-care, work and leisure, as appropriate to their individual needs. Activities depend on individual preferences and can include a variety of community based and educational choices.
People come to Coulby Lodge for different reasons; they may require more support than can be provided in a supported living setting, or they may be moving along their care pathway from a hospital placement. Whatever the reason, Coulby Lodge is adaptable to the service users living there, both in the living environment, which can be changed to suit individual need, but also in the support provided, as well as length and purpose of stay. Some people will only stay for a short period of time as they gain the skills they need to feel confident in their abilities to look after themselves when they move on to a supported living environment. Others will stay for a longer period of time.
Within Coulby Lodge we follow the five key principles that form our model of care, Personal PATHS.
1. Positive Behaviour Support (PBS)
2. Appreciative Inquiry Methodology
3. Therapeutic Outcomes
4. Healthy Lifestyles
5. Safe Services
We believe that we must be fully accountable to the people we support, their families and to those who commission services on their behalf. Personal PATHS model of care is our unique way of supporting this.
