Case Study 5

Hollyhurst is a Residential Care Home with Nursing for adults with learning disabilities

Based in Darlington, County Durham, Hollyhurst supports adults who are living with a learning disability and complex needs. The team can also support those with complex behaviours, as well as those who have associated mental or physical health needs.

Hollyhurst is a detached period property set in its own private gardens and accessed via a private tree lined drive. There are numerous living areas including a quiet lounge and two large communal lounges. Everyone has their own bedroom, which they are encouraged to decorate and personalise to suit their own personal style.

There is also a supported life skills kitchen where people can plan and cook meals and snacks for themselves and their visitors. They can also spend time relaxing with their friends, family and other visitors in the family room. A computer room is open throughout the day so service users can access the internet and develop IT skills, staff are available for support with this. There is an aromatherapy room, where an aromatherapist visits once a fortnight to provide uplifting massage to service users. There is also a new multi-sensory room that can be used for relaxation or stimulation. The garden project provides opportunities for service users to participate in growing fresh vegetables in the greenhouse and allotment located in the grounds.

In addition to the main house, within the grounds we also have Hollyhurst Lodge, a bespoke self-contained service delivering 24 hours care and support for two individuals. This service was specially designed in partnership with local commissioners and community teams to provide personalised support for two people with complex needs. Each person has their own individual living /dining room and en-suite bedroom. There is a secure shared life skills kitchen for people to learn how to cook meals and snacks for themselves and their visitors.

Within walking distance is the market town of Darlington, which has many community facilities for people to access, including leisure centres, social clubs, shops, colleges, and work placement settings.

Hollyhurst has a dedicated Activity Coordinator who supports service users to have individual activity plans, which are created based on each person’s likes and hobbies. Their goal is to enable people to structure their time, build relationships and establish a balance between therapy, self-care, work and leisure, as appropriate to their individual needs.

People may come to Hollyhurst for many reasons. They may be moving on from a hospital setting or may require a service which can support them more than a residential placement. It may be that they are leaving a residential or home setting and require a little support during a difficult period in their life, or they could be on a pathway to a place of their own.

Our Model of Care

Within Hollyhurst we follow the five key principles that form our model of care, Personal PATHS.

1. Positive Behaviour Support (PBS)
2. Appreciative Inquiry Methodology
3. Therapeutic Outcomes
4. Healthy Lifestyles
5. Safe Services

We believe that we must be fully accountable to the people we support, their families and to those who commission services on their behalf. Personal PATHS model of care is our unique way of supporting this.
