Case Study 10

Trinity House is a Residential Care Home with Nursing for adults living with a learning disability

Based in Lockerbie, Trinity House offers care and support to adults with a learning disability and complex needs within a structured and therapeutic environment. The team can also support those with additional mental and physical health needs, as well as complex behaviours. There is good access to appropriate clinical and therapeutic specialist support to enable us to deliver creative, flexible and effective responses to individual need.

Trinity House is a detached period property set away from the main road. There is a choice of living areas, a bright dining room and an activities room. Everyone has their own en-suite bedroom and people are encouraged to personalise and decorate their room to make it their own. There is a life skills kitchen where people can plan and cook meals and snacks for themselves and their visitors. Outside there is an established garden with greenhouses where the green fingered can enjoy cultivating plants and flowers. There are also seating areas for relaxing with friends.

Trinity House offers many activities for the service users they support. These include regular fishing trips, sensory activities at sensory centres, hydrotherapy sessions, table tennis, athletics, football and community outings. Staff also run a photography group and weight management with input from their highly qualified chef team. Service users also have voluntary opportunities at the Sammy Ling centre, Mossburn animal sanctuary and local charity shops. In addition, the local college runs courses for living skills. Trinity House also has a “secret garden”; a piece of conservation land which was donated locally to the service users to manage and utilise. The secret garden hosts Trinity House’s annual firework display, as well as BBQs throughout the summer.

Trinity House has a dedicated Activity Coordinator who supports service users to have individual activity plans, which are created based on each person’s likes and hobbies. Within walking distance is the town of Lockerbie, which has many community facilities for people to access including leisure centres, social clubs, shops, colleges, and work placement settings. Lockerbie is situated close to the larger town of Dumfries which has many more amenities.

The team at Trinity House are focused on ensuring that service users are part of their local community and can access education, employment and other opportunities to meet their needs. We encourage and support people to undertake vocational training courses which may help them to find supported employment in the future. One person at Trinity House has been awarded a SVQ level 1 in Horticulture, whilst another attained a certificate in ‘Healthy Eating Choices’ and now enjoys advising others about how to improve their diet.

The people at Trinity House may be stepping down from hospital provision or may be people who need a more robust package of care than can be provided in supported living. They may also be people who are on a journey towards a place of their own. Trinity House includes a self-contained two-bed bungalow, which supports those making the move towards more independent living.

Our Model of Care

Within Trinity House we follow the five key principles that form our model of care, Personal PATHS.

1. Positive Behaviour Support (PBS)
2. Appreciative Inquiry Methodology
3. Therapeutic Outcomes
4. Healthy Lifestyles
5. Safe Services

We believe that we must be fully accountable to the people we support, their families and to those who commission services on their behalf. Personal PATHS model of care is our unique way of supporting this.
